You continue through until you either defeat the final boss or die, and then regardless you usually unlock extra features (or difficulty levels or characters) and then you can play the game again if you would like. When you start the game you start from the first level and as you progress through you end up fighting enemies, bosses, finding power ups, and buying new abilities. It's a little hard to explain, but basically it's a game sub-genre that plays similarly to arcade games.
